Welcome to the Pediatric Pulmonary and Sleep Clinic at Hassenfeld Children’s Hospital. You are the most important partner in your child’s care.  Below are important instructions for your child’s medical visit.

 

Before your child’s visit:

  • Collect records from your child’s urgent care or emergency room visits, hospitalizations or other doctor appointments related to breathing problems.
  • Create a list of all your child’s medications.
  • Bring the records, the medication list and your child’s medications to your child’s medical visit. 
  • Arrive at least 15 minutes early to your appointment to complete registration papers.
  • If you will be more than 15 minutes late, please let the office know 24 hours in advance.  Otherwise, we may need to reschedule when you arrive. 

 

After your child’s visit:

  • Schedule a follow up appointment for your child when you check out.
  • Set up a breathing test (Pulmonary Function Test/PFT) as a separate appointment for the same day if the care team requests it.

 

How to contact our office when you are home:

  • If you need to reach our care team, call 212-263-5940, Option 4 (8am-5pm).
  • After 5pm, use On Call service for urgent concerns only if your child does not improve with a sick visit with your child’s pediatrician.
  • Please do not use “On call” services for medication refills, prior authorizations, appointments, lab results, form completions, etc.
  • Please use MyChart for non-urgent questions such as medication refills and non-urgent questions (team will respond within 3 business days (72 business hours)).
  • Contact your child’s pediatrician for routine/urgent sick visits unless our team offers different guidance.
  • If your child is having a medical emergency, call 911 right away.
